PREPARING FOR YOUR FIRST VISIT
Please plan to arrive at least 10 minutes prior to your appointment, which will take 30 minutes to 1 hour to complete.
In addition to familiarizing yourself with the information on our Web site, here are some things you can do to prepare for your first visit to our office:
- Write down questions you want to ask us in advance, so that we can be sure to fully answer them for you during your appointment.
- If you are seeing us for: a. Headache; TMJ-related pain or dysfunction; or head, neck or craniofacial pain; complete your TMJ Questionnaire as thoroughly as possible in advance of your visit. b. Snoring, Obstructive Sleep Apnea or CPAP intolerance, complete your Sleep Questionnaire as thoroughly as possible in advance of your visit. Note: If you have completed a sleep study (i.e., polysomnogram), please instruct your physician to fax a copy of it to our office in advance of your appointment. Our fax number is (304) 757-3535. If you prefer, you may also bring a copy of your sleep study with you.
- If you are currently using a sleep appliance, orthopedic stabilization or repositioning splint (i.e., TMJ appliance) or an orthodontic retainer, bring it with you to your appointment.
- Remember to bring your medical insurance card(s).
Headache, TMJ disorders and sleep disorders are medical conditions that can be managed effectively by dentists; therefore, you will qualify for covered benefits under your medical insurance policy.
For patients who are traveling a distance to see us, we are happy to provide driving directions plus a list of nearby hotels.
